112 The regimen tested in our study certainly does not appear to have an advantage in terms of response rate or survival over other published regimens. This was nevertheless a well tolerated regimen and easy to administer in an out-patient setting. The moderate results may be in part explained by the high-risk patients entered in this study, in particular patients with a performace status of 2 26% ; . In our previous randomized study only approximately 10% had a performance status of 2 [14]; these patients are often excluded in randomized studies in the US [15]. As a result of this patient selection we had eight early deaths, which were likely not related to the treatment but to the disease. Costs are becoming an important issue after the introduction of the newer anticancer agents in the treatment of solid tumors. Because the benefit of chemotherapy given in advanced NSCLC is marginal [4], it is important to include also a cost-effectiveness analysis and quality of life in the new generations of studies. In conclusion, our study investigated a novel combination of gemcitabine and paclitaxel in chemotherapy-naive advanced NSCLC patients. The response rate and survival in this multicenter study were comparable to that reported in similar patient populations. This regimen has now been elected as one of the arms of the current randomized trial of the EORTC Lung Cancer Cooperative Group, mainly based on its low toxicity profile and ease of administration. A randomized phase III trial in the first-line therapy of metastatic colorectal cancer demonstrated improvement in response, duration of response, and overall survival with the addition of bevacizumab to chemotherapy vs chemotherapy alone.38 It should be noted that recently, an increase in arterial thrombotic events has been reported in patients treated on trials across all diseases in patients who receive bevacizumab as compared to those who do not NCI communication, unpublished data, 2004 ; . A phase I II study of single-agent bevacizumab enrolled 75 patients with refractory metastatic breast cancer.39 In all patients, an objective response rate of 6.7% was confirmed, and 17% of patients were still being treated with response or stable disease at day 154 of assessment. Hypertension, nephrotic syndrome, headache, and thromboembolic disease were the main serious adverse events reported in this trial, which were similar in other trials of this agent. Subsequently, a randomized phase III trial of capecitabine vs capecitabine plus bevacizumab was reported with disappointing results.40 In this population of extensively pretreated patients with metastatic breast cancer, the primary endpoint of progression-free survival was not improved with the addition of bevacizumab, nor was survival. An increase in objective response rate was seen from 9% for capecitabine alone to 20% for the combination. Thirty-two patients were treated with paclitaxel plus gemcitabine. Twenty-five 78% ; received this therapy as third-line chemotherapy, six patients 19% ; as fourth-line, and one patient as fifth-line chemotherapy. All patients had progressed after HDCT. Most patients who achieved a CR to their HDCT also were treated with a subsequent 3 months of maintenance daily oral etoposide.20 Toxicity was as previously described.17, 18 The main toxicities were myelosuppression and peripheral neuropathy. There was no treatment-related mortality, and no patient stopped therapy due to toxicity. Four patients 12.5% ; achieved a PR; however, these were all of brief duration 2 to 6 months ; . Six patients 19% ; achieved a CR, and four patients remain continuously NED for more than 20, 40, 44, and 57 months from the start of paclitaxel plus gemcitabine, respectively. None of these four patients had subsequent resection of teratoma or carcinoma. An additional CR patient is currently NED more than 63 months from start of paclitaxel plus gemcitabine, but he has had two subsequent resections of carcinoma last surgery, March 2001 ; . The clinical details for the six CR patients are depicted in Table 1. All six patients received prior bleomycin etoposide cisplatin BEP ; and HDCT. They also all received a single course of vinblastine ifosfamide cisplatin VeIP ; before HDCT, as previously described.6 One patient Patient 3, Table 1 ; was treated as fifth-line therapy. Although he previously responded to all prior regimens, the prior remissions were all brief except for a 28-month CR with HDCT. He is now currently NED more than 44 months after initiation of paclitaxel plus gemcitabine. It is noteworthy that these six patients had relatively minimal tumor burden. Two patients, both of whom are NED, only had elevated markers with normal radiographic studies. Three other patients had elevated markers and progressive small-volume pulmonary metastases. The median survival for the entire group of 32 patients was 8.0 months 95% CI, 5.0 to 12.0 months; range, 2 to 63 months ; .
