Injectable non-insulins for blood glucose control: Symlin and Byetta Two new injectable non-insulin drugs are available for use by people with diabetes, both Type 1 and Type 2. They have totally different mechanisms of action. Symlin pramlintide acetate ; injection is indicated for both Type 1 and Type 2 diabetes, as an adjunct treatment in patients who use mealtime insulin therapy and have failed to achieve desired glucose control despite optimal insulin therapy, with or without a concurrent sulfonylurea and or metformin [11]. It is a synthetic analog of human amylin, a 37-amino acid peptide neurohormone made in pancreatic beta cells the same cells that make insulin ; [12]. Insulin and amylin concentrations normally increase after meals, while glucagon levels decrease. Amylin slows gastric emptying, suppresses postprandial plasma glucagon and endogenous glucose output from the liver, and increases satiety, helping keep blood glucose levels under control. Since Symlin slows gastric emptying, it can decrease absorption of orally administered drugs, which should not be taken for 1 hour before or 2 hours after injection. It is also not recommended for patients taking other agents that slow gastrointestinal motility e.g. anticholinergics ; , or slow intestinal absorption of nutrients [acarbose Precose ; , miglitol Glyset ; ] [13]. Mixing pramlintide in the same syringe with insulin may change its pharmacokinetics, although one study showed no clinically significant effect when 30 mcg of pramlintide was mixed in the same syringe with various doses of short- and long-acting insulin [14]. A single 5-mL vial of Symlin costs US and contains fifty does at 60 mcg, which is less than US.60 per dose. In a highly motivated patient who can tolerate the nausea and is willing to endure 2 to 4 more shots daily, it can offer additional tight glucose control, but the insulin dose must be managed to avoid hypoglycemia. The concept of the incretin effect developed from an observation that insulin responses to oral glucose exceeded those measured after intravenous administration of equivalent amounts of glucose [15, 16]. This led to the discovery that the gut-derived factors, or incretins, were released after nutrient entry into the stomach and stimulated insulin secretion. In fact, this oral incretin effect is responsible for about 50% to 70% of the insulin response to oral glucose in healthy individuals [17]. The two predominant incretin hormones are glucose-dependent insulinotropic polypeptide gastric inhibitory polypeptide, or GIP ; , and glucagon-like peptide-1 GLP-1 and minoxidil.
Icant reductions in fasting plasma glucose FPG ; and postprandial plasma glucose at each test meal timepoint, and for the glucose AUC area under the curve ; measured over the 2 h after the start of the meal. There were no significant differences between the glucose treatment effects seen in the miglitol 50 mg group and in the glyburide group except at the 60-min postprandial timepoint, at which time the reduction in the miglitol 50 mg group was significantly greater than in the glyburide group. Fig. 3A is a graph of the mean plasma glucose changes from baseline at each test meal timepoint in each treatment group, at the 1-yr endpoint. Though the reductions in the miglitol 50 mg group exceeded those in the miglitol 25 mg group at each timepoint, these differences were not significant, nor were the glucose AUC reductions in the three active treatment groups significantly different, although the greatest AUC reduction occurred in the miglitol 50 mg group. Postprandial plasma glucose excursions above fasting were reduced most in the miglitol groups, with very little change in the glyburide group, over the year of treatment. Fasting and postprandial insulin levels in the placebo group changed little from baseline throughout the course of the study. Figure 3B displays the mean change from baseline in serum insulin at the 1-yr test meal timepoints for each treatment group. Fasting insulin levels were essentially unchanged in either miglitol group, although reductions from placebo were seen in the miglitol groups at the three postprandial timepoints. These placebo-subtracted differences were significant at 60 min for the miglitol 25 and 50 mg groups, and at 90 min for the 50 mg group. On the other hand, significant increases in serum insulin were seen at the fasting and at all postprandial timepoints in the glyburide group compared with each of the other groups. Glyburide-associated postprandial insulin changes increased progressively from fasting through 120 min after the meal. There were no significant differences at any timepoint between the serum insulin changes seen in the two miglitol groups. Little change between treatment groups was evident in any fasting lipid parameter, in postprandial triglyceride levels, or in the normalized urinary excretion of glucose or.

STRONG PERFORMANCE CONTINUES Our 2005 business performance was strong and met expectations. The combination of proper nutrition, weight bearing exercise, and metformin may ultimately be most effective.16 The challenge for patients and practitioners regarding lifestyle intervention is to determine which dietary and exercise approaches are the most effective. Physical activity impacts body composition and makes the body respond better to insulin. Physical activity helps muscle cells use sugar for fuel.
