The Unit has pioneered a method of measuring total activity, including all these tiny insensible movements, by using a small combined heart rate and movement sensor that is worn by volunteers while the data are logged. "We have shown that total activity correlates with the risk of metabolic diseases such as diabetes, and is inversely related to weight gain, " he says. "This may seem obvious, but it hadn't been proven. It's important to study this, as it can direct our preventive efforts." The activity monitor is being used in different age groups, from as young as five, in a series of studies. An analysis of the European Youth Heart Study, published by the Unit in 2004, showed that metabolic diseases were commonest in the inactive, and that this was independent of obesity and fitness. In other words, activity has potential health benefits over and above its effects on weight. "We want to understand the causes of the growth in disorders such as diabetes, making comparisons within and between populations where risk varies. Currently we are measuring activity in countries as diverse as India, Cameroon, Greenland, and Kenya, trying to amass information about changing activities levels in countries undergoing Westernisation, " Dr Wareham says. "It is thought that one of the characteristics of a Westernised lifestyle is inactivity, but the evidence base is currently thin." These studies are examples of the larger number being run by the Unit in an effort to understand the rapidly rising rates of obesity and type 2 diabetes. Dr Wareham is a physician specialising in this area, who, in addition to seeing patients in the Addenbrooke's diabetes clinic, chairs the Cambridge Diabetes Managed Care Network. His background is in public health medicine, for which he trained in London, Harvard and Cambridge. These interests come together in a wide-ranging portfolio of programmes at the Unit, ranging from the genetics of metabolic disease to behaviour change. Dr Wareham is a scrupulous gatherer and winnower of information, not easily impressed by the latest headline story about diet or activity. "The aim of the Unit is to use epidemiological methods to study the determinants of diabetes and obesity and to inform preventive action. This needs to be built on a solid platform of research." The genetic studies are carried out in collaboration with the Sanger Institute Barroso ; and the University of Cambridge O'Rahilly ; through the Genetics of Energy Metabolism GEM ; consortium. The aim is to discover how genes and lifestyle conspire to lead to diabetes and obesity. Large case-control studies, including 10, 000 cases and 20, 000 controls, are being amassed by the Unit to study the genetic basis of these diseases. The analysis of the results is not always straightforward. "Being able to distinguish the true findings is the big challenge and requires studies of sufficient scale, " Dr Wareham says. Clues about how genes and lifestyles interact will come from large casecontrol studies nested within population-based cohort studies. Wareham is leading efforts to build such a study within EPIC, the European Prospective Investigation into Cancer, a multinational cohort study of 500, 000 people designed to examine the association between lifestyle and chronic disease in mid-life. Ten thousand cases of incident diabetes and an equal number of healthy controls from EPIC are being identified. "We need a large study with 15 years of follow-up to get enough cases for the statistics to mean something, " Dr Wareham says. "We have now reached that point and the next few years are likely to be highly productive." At the other extreme of the Unit's research portfolio are efforts to identify preventive action. "In the past we have looked at individual behaviour, " he says. "But now we are trying to study the true environmental determinants things such as where you live, what access you have to green space, and so on." This research is being done in collaboration with geographers, using geographical information systems to study patterns of activity in Norfolk within the EPIC population and a new cohort of schoolchildren. The unit is currently based in the Strangeways Research Laboratory and at the Elsie Widdowson Laboratory named after a pioneer in the study of diet and nutrition but will move into a new building on the Addenbrooke's site. In addition to the MRC Unit, the building will house the University Institute of Metabolic Science and NHS clinical facilities for diabetes and obesity. Dr Wareham is looking forward to it. "We'll be within a multidisciplinary institute focussed on diabetes and obesity, but will still be part of the Cambridge Institute of Public Health. It is this combination of subject-specific and methodological expertise that is the key strength in Cambridge, " he says. 21.

Weber Pirlo In this immunocatalytic assay for alpha-amylase EC 3.2.1.1 ; of pancreatic origin, a highly specific monoclonal antibody coupled to plastic beads is used to extract pancreatic amyIase from samples, leaving salivary amylase in solution. The catalytic activity of the bound pancreatic amylase is then determined with blocked p-nitrophenyl maltoheptaoside as substrate. The method shows no cross-reactivity with salivary amylase, analytical recovery is 89-109% for pancreatic amylase, and interassay imprecision is 7.1-7.7%. We used the method to determine pancreatic amylase in serum and urine from healthy controls and different patient groups. The reference intervals for 34 supposedly healthy controls were: serum, 10-48 U L mean 27 U L urine, 20-435 U L mean 104 U L ; . Pseudomonas aeruginosa is a common nosocomial pathogen, particularly among immunocompromised patients. Carbapenems, mainly imipenem and meropenem, are potent agents for the treatment of infections due to multiresistant pseudomonads. These drugs have considerable -lactamase stability and overall have the broadest spectrum of activity compared with other -lactams 9 ; .
