For it three hundred and sixty channels. Leaving Armenia and entering the Matienian country, you have four stations; these passed you find yourself in Cissia, where eleven stations and 42 1 2 parasangs bring you to another navigable stream, the Choaspes, on the banks of which the city of Susa is built. Thus the entire number of the stations is raised to one hundred and eleven; and so many are in fact the resting-places that one finds between Sardis and Susa. If then the royal road be measured aright, and the parasang equals, as it does, thirty furlongs, the whole distance from Sardis to the palace of Memnon as it is called ; , amounting thus to 450 parasangs, would be 13, 500 furlongs. Travelling then at the rate of 150 furlongs a day, one will take exactly ninety days to perform the journey. Thus when Aristagoras the Milesian told Cleomenes the Lacedaemonian that it was a three months' journey from the sea up to the king, he said no more than the truth. The exact distance if any one desires still greater accuracy ; is somewhat.
Colored in a manner which would certainly have astonished that hero, if ever he happened to meet with its like. On a rough bench in the corner, a couple of woolly-headed boys, with glistening black eyes and fat shining cheeks, were busy in superintending the first walking operations of the baby, which, as is usually the case, consisted in getting up on its feet, balancing a moment, and then tumbling down, --each successive failure being violently cheered, as something decidedly clever. A table, somewhat rheumatic in its limbs, was drawn out in front of the fire, and covered with a cloth, displaying cups and saucers of a decidedly brilliant pattern, with other symptoms of an approaching meal. At this table was seated Uncle Tom, Mr. Shelby's best hand, who, as he is to the hero of our story, we must daguerreotype for our readers. He was a large, broadchested, powerfully-made man, of a full glossy black, and a face whose truly African features were characterized by an expression of grave and steady good sense, united with much kindliness and benevolence. There was something about his whole air self-respecting and dignified, yet united with a confiding and humble simplicity. He was very busily intent at this moment on a slate lying before him, on which he was carefully and slowly endeavoring to accomplish a copy of some letters, in which operation he was overlooked by young Mas'r George, a smart, bright boy of thirteen, who appeared fully to realize the dignity of his position as instructor. "Not that way, Uncle Tom, --not that way, " said he, briskly, as Uncle Tom laboriously brought up the tail of his g the wrong side out; "that makes a q, you see." "La sakes, now, does it?" said Uncle Tom, looking with a respectful, admiring air, as his young teacher flourishingly scrawled q's and g's innumerable for his edification; and then, taking the pencil in his big, heavy fingers, he patiently recommenced. "How easy white folks al'us does things!" said Aunt Chloe, pausing while she was greasing a griddle with a scrap of bacon on her.

This time the CSIR Foundation Day celebration was of very special significance to RRL, Jorhat as the laboratory was formally awarded the ISO certificate by the ISO auditing organisation Det Norske Veritas DNV ; , Kolkata. The function was largely attended by students, teachers, invited guests, prominent citizens and other public besides the members of the RRL, Jorhat family. Dr M. Hazarika, Director, Tocklai Experimental Station, Tea Research Association, Jorhat, was the Chief Guest and delivered the Foundation Day Lecture while Shri Sasim Chattopadhyay, Lead Auditor, Det Norske Veritas DNV ; , Kolkata, was the Guest of Honour. It is worth mentioning here that the International Quality Audit Organisation DNV of the Netherlands found the Quality Management System of RRL, Jorhat to conform to the Quality Management Standard and hence conferred the Management System Certificate on it. Welcoming the distinguished gathering, Dr P. G. Rao, Director, RRL, Jorhat spoke briefly on the achievements, glory and CSIR's contributions towards the nation building and also the role played by RRL, Jorhat in the economic, industrial and societal development of the country. Shri Sasim Chattopadhyay formally presented the ISO 9001: 2000 certificate to the Director, RRL, Jorhat which marked the beginning of ISO era in the laboratory. He spoke about the DNV, the ISO auditing organisation. He congratulated the RRL community for achieving the quality goal and also thanked the RRL management for selecting DNV for the assessment services and for being a partner in the quality movement in the region. Dr M. Hazarika gave a very illuminating and thought provoking lecture on `Science perception and management' and emphasized on the need for clarity of thinking, purpose and delivery mechanism depending on future priorities for achieving success. Science, as he said, brought ease to our life. But a good scientific idea does not necessarily transform into a commercial venture. The employees who have completed 25 years of CSIR service were presented the mementoes and employees who have retired during the period from September 2004 to August 2005 were presented shawls, watches and Sanman Patras as recognitions to their exemplary and meritorious services towards the growth and development of the laboratory. The function ended with a vote of thanks by Dr Anjan Baruah, a senior scientist. The ISO internal auditors were also awarded certificates for successfully receiving the Internal Auditor training from the experts.

VII. REIMBURSEMENT FOR BLOOD PRODUCTS AND SERVICES: PHYSICIAN OFFICES AND CLINICS The most appropriate CPT codes for blood transfusions and other common blood-related procedures that physicians may use to report services on the CMS-1500 form are included in Table 8. Physicians use the CMS-1500 claim form to report their work in all settings of care. Claims for physician services must be billed with the appropriate CPT code to describe the service. Medicare compensates physicians for their professional services and those payments may be made to the physician directly or to another entity where the physician has entered an assignment agreement, whereas private insurers may group physician services with the hospital payment. In most cases, private insurers will reimburse physicians for the professional component of the procedure as well for services rendered in the hospital outpatient or hospital inpatient settings. Medicare and some private insurers reimburse physicians for inpatient and outpatient care according to a resource-based relative value scale RBRVS ; fee schedule. Each CPT code has a fee associated with it that is based on the physician work, practice expenses, and liability costs required for that particular service the physician performs, and it is adjusted for the geographic variations associated with medical practice. Some procedures that have been modified but not changed in their description are identified with modifiers. For example, if a service involves equipment, it may be identified with a modifier to show the technical component where an actual service test is performed, and then a professional component to show the interpretation of the service test performed. Physicians may bill globally for services when provided in their office or may submit a claim only for the professional component of services provided in other settings. Physicians may enter into contracts--sometimes referred to as assignment agreements--with Medicare-enrolled entities such as hospitals ; to perform transfusions in settings other than their own offices. As noted above, previously these physicians could only submit claims for the professional component of their services. However, a recent instruction from the Medicare program available online at : cms.hhs.gov manuals pm trans R111CP , issued February 27, 2004 ; , now allows physicians to bill globally for both the technical and professional components of transfusion services where the physician has a contractual agreement with an entity enrolled in the Medicare program. Private insurers and Medicaid also may use a percent of allowable charges or negotiated rates to reimburse for physician services. The allowable charge is the maximum amount an insurer will allow when paying for a specified supply or service. Typically, the allowed amount is the lower of the provider's submitted charge or the payer's own maximum fee schedule amount. If the payer does not have a fee schedule, the allowed charge usually is set at some percentage of the provider's submitted charge. A negotiated rate is insurer specific and dovonex.

INTRODUCTION It has long been speculated that membrane active peptides may self-assemble into supramolecular structures within fluid membranes. The best known example is alamethicin. Both its crystalline structure Fox and Richards, 1982 ; and its single-channel conduction behaviors Bauman and Mueller, 1974; Latorre and Alvarez, 1981 ; suggested that alamethicin monomers assemble in the barrel-stave fashion to form transmembrane pores. Other peptides, such as melittin and magainin, are also likely to form pores in lipid bilayers as suggested by ion conduction Tosteson and Tosteson, 1981; Duclohier et al., 1989; Juretic et al., 1994 ; , leakage experiments Matsuzaki et al., 1996, 1997; Ladokhin, 1997 ; , and other evidence Dufourcq et al., 1986 ; . However, there has not been an experimental method capable of directly observing such oligomeric peptide structures in membranes. A few years ago we successfully detected membrane pores with neutron in-plane scattering He et al., 1995 ; . Neutron detects structures within the membrane where the neutron scattering-length densities are different from that of a pure lipid bilayer. In particular, use of D2O makes water-filled pores within the membrane stand out against the lipid background. In contrast, peptide pores in fluid membranes, with the exception of gramicidin, are difficult to detect by x-ray. ; This method allowed us to detect and measure the size of the alamethicin and magainin pores He et al., 1995, 1996; Ludtke et al., 1996 ; . However, to fully characterize these supramolecular structures in membranes, the diffraction data must cover all the reciprocal space, including the momentum transfer q projected out-of-plane as well as in-plane. Normally, diffraction measurement in a two-dimensional q space is a time-consuming process. In a previous paper Yang et al., 1998 ; , we showed a simple method of recording the complete pattern using existing small-angle neutron scattering facilities. By this method, a complete diffraction pattern of a typical peptide-in-membrane sample can be obtained within one hour at the Cold Neutron Research Facility of the National Institute of Standards and Technology Gaithersburg, MD ; . In this paper, we present the method for analyzing such diffraction patterns. When multilayers of membranes are fully hydrated, the individual bilayers are well separated by intercalating water layers, so that the in-pane positions of embedded objects, such as membrane pores, are uncorrelated between bilayers. In such cases, the qz the out-of-plane q component ; dependence of the scattering pattern is governed solely by the form factor of the scattering objects. The structural information for the lateral organization can be obtained by inplane scattering, as we have shown previously He et al., 1995, 1996; Ludtke et al., 1996 ; . However, some of the most intriguing phenomena of peptide-membrane interactions manifest themselves in less than fully hydrated conditions. In many cases, when the multilayer samples were underhydrated, the diffraction patterns showed that the inplane positions of pores became correlated between bilayers. This indicated a lateral repulsion between pores residing in neighboring bilayers. This phenomenon of lateral force between embedded objects in two neighboring bilayers has not been observed before. In very low hydrations or low temperature, strong correlations order the pores into a quasicrystalline, thereby allowing for the possibility of investigating the supramolecular structures with highorder diffraction.

Every technical illustration is done in a perspective. Usually, you will employ the perspective that best suits the information you wish to communicate within the depicted situation. Individuals who are new to technical illustration are confronted with several questions when choosing the appropriate perspective. Quite often, they try to depict a body as realistically as possible. To achieve this, they choose a vanishing point perspective for example, true perspective ; . However, as a general rule, you can say the more natural the perspective is, the more complex it is to include it in the illustration. The reason for this is the perspective reduction used. Sighted people know from experience that an object appears smaller at a distance than it does close up. It is also true that lengths are reduced as they move further from the observer's eye. This has two effects: Creation of a true perspective is difficult. When creating a true perspective manually, you cannot work with real measurements. You and dulcolax.
A. The descriptor for HCPCS code G0244 is changed to read: Observation care provided by a facility to a patient with CHF, chest pain or asthma, minimum eight hours. The new descriptor clarifies that separate payment will be made for observation services only when a minimum of eight hours of care have been provided to the beneficiary. Hospitals should report the number of hours the outpatient is in observation status. b. To receive separate payment for HCPCS code G0244, hospitals are required to report a qualifying ICD9-CM diagnosis code for 1 ; Congestive Heart Failure CHF ; , 2 ; chest pain or 3 ; asthma, as one of the following: Admitting Diagnosis Reason for Patient Visit, or Principal Diagnosis. The list of ICD-9-CM codes is published in the 2005 OPPS final rule. The code must be reported in the Admitting Diagnosis Reason for Patient Visit field form locator 76 or its electronic equivalent ; or the Principal Diagnosis field form locator 67 or its electronic equivalent ; to qualify for separate payment for observation services. c. Observation time begins at the clock time documented in the patient's medical record, which coincides with the time the patient is placed in a bed for initiating observation care in accordance with a physician's order. d. Observation time ends either when the patient is discharged from the hospital or is admitted as an inpatient. The time when a patient is "discharged" from observation status is the clock time when all clinical or medical interventions have been completed, including any necessary follow-up care furnished by hospital staff and physicians that may take place after a physician has ordered that the patient be released or admitted as an inpatient. However, observation care does not include time spent by the patient in the hospital subsequent to the conclusion of therapeutic, clinical, or medical interventions, such as time spent waiting for transportation to go home. 5. Billing for Drugs, Biologicals, and Radiopharmaceuticals a. New Dosage Descriptors for Certain Drugs and Biologicals Hospitals are strongly encouraged to report charges for all Drugs, Biologicals, and Radiopharmaceuticals, regardless of whether the items are paid separately or packaged, using the correct HCPCS codes for the items used. It is also of great importance that hospitals billing for these products make certain that the reported unit of service is consistent with the quantity of a drug, biological, or radiopharmaceutical that was actually administered to the patient. For 2005, several HCPCS codes for drugs and biologicals have undergone changes in their HCPCS code descriptors. Hospitals should be reminded that they should bill for units of service consistent with the dosages contained in the new long descriptors. The affected HCPCS codes are listed in Table 5 and doral.
