PARAMOUNT ELITE PREFERRED BRAND LIST This Preferred Brand List includes brand name prescription drugs available at the preferred brand copayment level, where a generic drug is not available. When a preferred brand drug becomes generically available, the brand drug will no longer be offered at the preferred brand copayment level. The generic version will be offered at the generic copayment level. The branded version, called a multi-source brand, will be offered at a higher copayment level according to the member's benefits. This list has been designed to inform you and your doctor about medications that are covered at moderate copayments. You can save additional money by requesting a generic product. The lowest copayment is applied to all covered generic drugs. Some common generics are also included on this list. These will be indicated by the letter "G" and will appear in lower case text. Some drugs have certain restrictions such as Step Therapy, Prior Authorization, and Quantity Limits. Paramount reserves the right to implement these restrictions for drugs on this list. The main aims of the action plan are to perfect the Free Trade Area6, to harmonise tariffs7, to reach regional economic agreements8, to facilitate trade9, to provide for mutual recognition of health records, to establish an agricultural trade policy, and to strive to institute joint or at least co-ordinated negotiation of various free trade agreements with other countries -special importance was attached to initiating negotiations on a free trade agreement with the United States10. In the case of Nicaragua, simply announcing this was sufficient to reactivate the mechanisms of participation and discussion amongst civil society, breathing new life into the dialogue with producer and consumer groups. Another aspect worth highlighting in this memorandum and by way of introduction, is that Nicaraguan legislation, in terms of its constitution, laws, agreements and ministerial regulations, formally supports a strongly-worded principle that encourages civil society participation in economic affairs. We should admit from the outset that these formal legislative declarations are not always put into practice. Without underestimating the important progress made so far, one of Nicaragua's greatest challenges in formulating trade policy is precisely to improve and deepen its capacity to put flexible, transparent, efficient and dynamic mechanisms and processes into systematic operation in its relations with civil society. This must be accomplished if the ideas expressed in terms of political will and legislation are to become a part of real life. The author will use the term "civil society" in this document to mean all organised groups in Nicaraguan society other than the government and other state and local authorities ; that represent certain interests and sectors and that aspire to play an active role in policies related to any aspect of national development.
Oscillations, which were originally observed in intracellular voltage recordings in pyramidal neurons and termed "giant depolarizing potentials" GDPs ; Ben-Ari et al., 1989; Leinekugel et al., 2002 ; . This type of synchronous network event is thought to be involved in the maturation of neuronal circuits Feller, 1999; Penn and Shatz, 1999; Katz and Crowley, 2002 ; . In this work, we show that although it has no or very little effect on single sIPSCs in early postnatal hippocampal slices, blocking GAT-1 leads to a marked enhancement of GABAA receptor GABAAR ; -mediated transmission during GDPs and crixivan.

And nonexpressed genes. This analysis resulted in the identification of 716 rhythmic genes of 6, 818 genes expressed in the liver 10.5% ; and 267 rhythmic genes of 7, 824 genes expressed in skeletal muscle 3.4% ; . The liver and skeletal muscle values are consistent with past reports 19, 20 ; regarding the percentage of rhythmically expressed genes in these tissues. As in previous studies, our results indicate that only a small number of genes are rhythmically expressed in more than one tissue, even when no intensity value cutoff is imposed. Only 57 genes had an MMC value 0.2 in both tissues sampled, many of which have previously been shown to be circadian SI Table 3 ; . These genes include core pacemaker components Per2 and Cry2 ; , their regulators Nr1d1 Rev-erb and Stra13 Dec1 ; , and their immediate downstream targets Dbp and Tef ; . Using information derived from COSOPT analysis, we determined the phase distribution of rhythmic genes Fig. 1C ; . In liver, we found the largest clusters of rhythmic genes at CT06 and CT18, 6 h after the onset of subjective day and subjective night, respectively. These data are in agreement with previous studies on the phase distribution of gene expression in the liver and likely represent physiological demands on metabolism and catabolism 19 ; . In contrast, in muscle we identified a single large-phase cluster at CT18. As CT18 represents the midpoint of the active phase, 6 h after the onset of activity, this peak may be activity-induced. It has previously been shown that resistance exercise can directly affect expression levels of key clock components and downstream targets in muscle 34 our data suggest that locomotor activity may act to phase-coordinate expression of rhythmic genes in skeletal muscle. We used GeneSpring software to sort highly significant rhyth0.1 ; into functional categories mically expressed genes MMC and hand-annotated the list for accuracy. In both tissues, genes and cubicin.
Eye and is on cosopt and iopidine eye drops twice daily. Victoria wears rigid gas permeable lenses and reading glasses. Her corrected vision is presently 20 60 in her left eye and 20 100 in her right eye. Contact lenses and eye drops are a regular part of our routine. Victoria is an active and outgoing six year old, who doesn't let anything get in her way. Her strong will and determination will help her achieve her goals and accomplish great things in life! Our third child, Jessica was born in November of 1999. Her birth was assisted by a midwife who does a red eye reflex test as a regular part of her birth examinations. Therefore, Jessica's cataracts were discovered within the first few hours of life. Although at first I thought I couldn't possibly go through this again, we were much more prepared for the challenges of eye drops and contact lenses the second time around. Jessica's first cataract was removed at nine days of age. The operation went well, but she did not respond well to the general anesthetic. Therefore, Jessica was kept on oxygen for the next two days, and the cataract in her other eye was removed at thirteen days of age. She wears Silsoft lenses, and is generally somewhat compliant regarding insertion and removal, although she tends to lose a lot of lenses! Her vision is developing normally and she is a healthy and active one year old. As I reflect on the last six years of our life, I aware that we have many things for which to be thankful. We are blessed to live in a country in which our children have access to necessary medical treatment. We are very fortunate to live in the city of Toronto with such a wonderful medical facility as Sick Children's Hospital. We have skilled doctors including Dr. Alex Levin, Dr. Brent and Dr. Terry Lewis to whom we offer much gratitude, recognizing that it is due to their commitment to improving children's vision that our girls have been able to attain their present visual acuity, allowing them to see the beauty of this world in detail. I grateful for a wonderful employer who has been understanding and flexible with regards to appointments for the girls. Mostly, we are thankful for the prayers and support of our family and friends. By Jan Ong.

Over two-thirds of all poor households live in rural areas where persistent and widespread poverty is the norm. More than half of all rural households fall below the 1995 official rural poverty line of P 8, 000 US$ 310 ; per capita. A continuing high population growth rate 2.3% p.a. ; , increasing landlessness and scarcity of land, and inadequate growth of the agricultural sector, call for a improved strategy for managing this economic sector of society. Some 75% of households cultivate 1 hectare or less, which, at full development, would only provide incomes ranging from US$ 393 to US$ 785. Some 40% of farms are cultivated by leaseholders. The size of landholding and issue of landlessness is determined by institutional factors such as rules of inheritance, where family property is equally divided among all siblings, and the Philippine Civil Code, whereby property is jointly owned by the conjugal unit. Grger et al. ples from the Rodna horst, situated in the East Carpathians Bucovinvian nappes ; . This area was affected by Cretaceous medium- to low-grade metamorphism, followed by post-collisional exhumation and renewed moderate thermal overprint due to the deposition of Eocene to Early Miocene sediments. Based on paleostress analyses of mesoscale structures, three main tectonic phases can be disdinguished in the study area, all of which are postdate the earliest Miocene Aquitanian, 20.5 Ma, Fig. 2 ; . In late Early Miocene Burdigalian ; the Pienide nappes, nonmetamorphic flysch series, were emplaced onto the Paleogene to Early Miocene sedimentary cover of the Bucovinian nappes. Emplacement of the Pienides is consistently SE-directed and dominated by brittle thrusting while folding is of subordinate importance only. The curved map appearance of the thrust contact is due to lateral ramps and tear-off faults, accentuated by later folding. This early Miocene thrust contact was sinistrally offset later on along the Bogdan-Voda fault Fig. 1 ; . Activity along the Bogdan-Voda fault started in Middle Miocene times as a sinistral transpressive fault related to the formation of open NW-SE trending folds in the sedimentary units. In a next stage late Middle Miocene to Late Miocene ; kinematics along the coevally active Bogdan-Voda fault and its eastern prolongation, the Dragos-Voda fault, formed one through going fault, characterised by sinistral transtension. Towards the east the left lateral offset along the main fault is continuously reduced by coevally active SW-NE trending normal faults e.g. Greben fault, Fig. 1 ; , thus terminating the DragosVoda fault in an extensional horsetail.

OM showed GR and G6PD activities simi lar to LM. The reason for the decline in enzyme activities at 320 ppm vitamin E supplementation remains to be defined. The erythrocyte glutathione peroxidase system has been extensively studied both because of the ease of repeated sampling and the relative simplicity of the system in this tissue only the hexose phosphate shunt produces NADPH ; . However, the erythrocyte enzymes showed neither the phenotype nor E effects of their hepatic counterparts. In this study the erythrocyte proved to be a poor indicator of hepatic response. Significant differences were found in the proportion of various fatty acids in adipose tissue with increased intake of E. The in crease in proportion of linoleic acid in the adipose tissue with increased intake of vitamin E is consistent with the antiperoxidant effects of E. Possibly even greater differences would have been observed if analysis of membrane fatty acids had been conducted rather than total tissue fatty acid analysis. In view of the possible detrimental ef fects of lipid peroxidation on aging 3 ; , membrane integrity 32-34 ; and carcinogenesis 35 ; , coupled with the observed differences in tissue FSAO content and MDA between LM and OM, a closer evalu ation of the interaction between dietary E and tissue peroxidation in obese subjects appears warranted and cosopt.
