The group of 27 orphan compounds introduced from 1990 to 1994 included six cancer treatments the largest indication category ; . All of these six drugs also received a priority drug rating from the FDA as well as orphan drug designation. The six cancer drug introductions receiving orphan drug approval over the 1990-1994 period were altretamine, cladribine, fludarabine phosphate, idarubicin, pentostatin, and teniposide. ; These six orphan therapies had peak global sales that ranged from million to 3 million. The median and mean global peak sales for this set of orphan cancer treatments were million and million, respectively. The Orphan Drug Act clearly has been an important stimulant of new cancer treatments for small patient populations with correspondingly modest levels of sales. Orphan drugs can realize a positive return on investment with smaller sales levels than can non-orphan products given their very different economics. First, as discussed, they have much smaller up-front R&D costs, with typically smaller sized clinical trials and the special tax credits. In addition, orphan drugs often have fewer competitors and are typically prescribed primarily by specialists. Hence, they tend to have lower promotional and distribution costs. Commercial Significance of Oncology Drugs As in the case of other therapeutic categories, the distribution of sales for cancer drugs is highly skewed. Although the oncology class includes a number of orphan drug compounds, it also has many drugs that are in the top ranks of all drug products. These are products with annual worldwide sales in excess of billion. With an increased knowledge of the molecular basis of cancer, the oncology class has been characterized in recent years by the introduction of therapeutically important monoclonal antibodies and other targeted pharmaceutical agents. These include rituximab, trastuzumab, imatinib, and bevacuzimab. All of these drugs have reached the market between 1997 and 2004. These are now among the leading drug therapies ranked in terms of sales. The oncology drug class is the fastest growing therapeutic category of all the major drug classes in terms of market sales. The rapid growth of the cancer area is reflected in the annual sales of the top 200 pharmaceuticals. Med Ad News chronicles this each year. Our analyses of development candidates and marketed products abstracts from the supportive care products used to ameliorate adverse effects such as anemia, neutropenia, and nausea and vomiting that are frequently experienced with treatments of diseases such as cancer and AIDS. It is worth noting that the leading biologically derived drugs for anemia and neutropenia have also experienced rapid growth in recent years. In a controlled trial, approximately two thirds of patients with acute gout had a response to colchicine within hours when the drug was initiated within the first 24 hours after the onset of arthritis.24 Episodes of acute gout may be curtailed if patients have a supply of oral colchicine available for self-treatment e.g., one 0.6-mg tablet every hour for up to three hours, for a maximum of three tablets ; at the first sign of recurrent arthritis.15 Use of more extended regimens of oral colchicine as a primary treatment for acute gouty arthritis is generally unwarranted.15 The therapeutic ratio of benefits to adverse effects is usually poorer for colchicine than for other treatments15 Table 1 ; . The use of intravenous colchicine is discouraged, because this approach has been associated with substantial complications15 and at least 20 deaths.25 Serious adverse effects, including bone marrow suppression, may occur even with the use of low doses less than 2 mg ; of intravenous colchicine in patients with renal insufficiency and hepatobiliary obstruction, particularly in those over 70 years of age, since these conditions and aging impair the elimination of colchicine.15, 25 Colchicine cannot be extracted by dialysis, and treatment of established colchicine intoxication is primarily based on supportive care. Treatment of colchicine-induced neutropenia with granulocyte colony-stimulating factor is a rational therapeutic intervention but has not been assessed in a controlled trial.26 Early, antibody-based treatment of a recognized colchicine overdose to avoid the toxic effects of the drug remains investigational.27.

Walking along the winter streets by the campus post office, furious that John claimed he would not necessarily defend a dreadful criminal. Who did he think he was? Defense of the defenseless, the guilty, and even the indefensible was already my religion. Now of course our basic argument did not apply directly to public defenders, because they don't choose their clients. But there is an analogous attitude that John and I also disputed. Basically, I believed that public defending was a sacred duty that requires a certain soul-set and selflessness that only a special class of people is capable of achieving: a mindset that values freedom over justice any day. In this vein, long after I had become an academic, I reviewed a book about public defending at my old office in the District of Columbia: "How Can You Defend Those People?": The Making of a Criminal Lawyer, by James Kunen.135 While acknowledging the well-told stories, I took the author to task for betraying the inner circle of defenders, for speaking about his clients from an ironic distance, for staying only two-and-a-half years not long enough to repay the training he received ; , and for using the job to get material for his book. John was first puzzled and then somewhat insulted by my approach, and he irritated me in turn by suggesting that I must have some secret grudge against James Kunen. None--other than his failure to meet my standard of single-minded devotion. Today, two decades later, I wouldn't approach that review the same way but would instead welcome the time the author gave to the movement, his sympathetic portrayal of the work, his willingness to do it. In writing this piece about John's experience, I have realized that it is not only unnecessary to hold my belief system in order to defend, but that an attitudinal litmus test will stand in the way of ever realizing Gideon's promise. Furthermore, it requires only small amounts of material while allowing biophysical activity testing of surfactant in a leak-proof surfactometer. Finally, because application of this technique to a comparative study of spread and adsorbed film showed that differences in surface activity are small, the developed method will be useful for mechanistic studies of surfactant protein-phospholipid interactions of spread film and comparative studies of adsorbed and spread film and aranesp.
